Job Description:
We are seeking a Lead Care Coordinator (Registered Nurse) to
join the Delaware Valley team in a hybrid role! The Lead Care
Coordinator, RN serves as a resource to the care coordination team.
The Lead Care Coordinator RN assists the Clinical Supervisor of
Care Coordination and the Manager of Care Coordination/Community
Health Worker by providing clinical oversight and direction that
ensures the safety and quality of the care provided to patients and
families. The Lead Care Coordinator RN is responsible for promoting
and advancing the professional development of all Care Coordinators
in the ambulatory setting through role modeling, mentoring, and
providing feedback.The lead care coordinator will assist with
training new care coordinators, conducting interviews, implementing
new workflows/projects, and ensuring effective communication
between case management, care management, and other departments.
Run reports to assess KPIs are being met, run gaps in care and
referral reports to address gaps in care and promote timely access
to appropriate care, increasing the utilization of preventative
care and healthy behaviors to support the Nemours priority of whole
child healthThe Lead RN Care Coordinator will coordinate ambulatory
team-based care to provide comprehensive support and guidance to
patients with chronic diseases that require specialty care through
effective partnerships with the PCP, community resources, medical
professionals, the patient's medical home team, and the Nemours
organization to enhance patients' access to education, counseling,
and ongoing support beyond the clinical setting.They will perform
all essential and non-essential job functions of the Specialty Care
Coordinator job description, in addition to duties outlined below
or to be assigned. The ideal candidate will possess excellent
leadership and communication skills, strong organizational
abilities, and a passion for improving the health and well-being of
individuals with chronic healthcare
needs.Responsibilities:Responsible for executing all core functions
outlined in the Specialty Care Coordinator job description, as
detailed in the attached document.Actively participates in the
recruitment process, including conducting interviews and assessment
of candidates for care coordinator positions to ensure alignment
with organizational goals and values.Coordinates and oversees the
comprehensive orientation program for newly hired care
coordinators, ensuring they successfully achieve established
performance goals within the first 90 days of employment.Acts as a
knowledgeable resource for care coordinators, providing guidance
and support to ensure adherence to relevant policies, procedures,
and best practices in patient care delivery.Monitors and verifies
the completion of annual competency assessments for care
coordinators, collaborating closely with their direct supervisors
and the Manager to track progress and ensure compliance.Organizes
and facilitates monthly care coordinator meetings, ensuring the
dissemination of key information, updates, and training
opportunities relevant to the team's success.Conducts regular
assessments and monitoring of key performance indicators (KPIs) and
HEDIS measures, analyzing trends and outcomes to drive continuous
improvement in patient care.Undertakes additional responsibilities
as assigned by management to support the overall objectives and
operations of the care coordination team.Qualifications:Bachelors
degree in nursing RequiredMinimum of 3-5 years of experience in
care coordination, preferably in a pediatric healthcare setting
requiredActive Delaware of compact RN license
requiredInpatient/outpatient or Specialty nursing, Care, Case, or
Care Management experience preferredCPN preferredby Jobble
